What does this mean for you and your staff?
- Your benefit entitlements will remain unchanged.
- Annual and lifetime maximums are the same.
- Your health and dental claims history for you and your dependents will be transferred to Medavie from the previous carrier.
- Submit all claims to the new carrier after August 1.
- New benefit cards will be sent to your home address.
- A new life insurance beneficiary form for Manulife has been issued for completion. Ink signature is required but can be sent back electronically.
Encourage people to continue to fill out the Manulife Beneficiary Forms. Instructions have been added to the FAQ on the intranet.
How do I fill out my Manulife Beneficiary Form?
- Keep the pre-populated Manulife policy number – this is new!
- Certificate number is your employee ID number
- As a resident of Ontario, your life insurance beneficiaries are revocable, and you do not need to complete that section.
- Unless you currently have additional optional or spousal life insurance, you do not need to complete that section.
- If you have more beneficiaries than space allows, you can include an additional sheet and list all names. Ensure to date and sign the attachment and return all documents together.
Benefit cards mailed the week of July 19
Benefit cards were put in the mail by Medavie Blue Cross on July 19 and being delivered via Canada Post. IF someone has an urgent claim submission prior to them receiving their benefit card, after August 1, the Total Rewards team can get them an electronic benefit card.
